Job summary
An exciting opportunity has arisen for a part time band 6 Nurse to develop skills in diabetes nursing. Supported by the Diabetes Specialist Nurse, Diabetes Nurse educators, Diabetes Nurse Associate and Diabetes Nurse Consultant, you will be working with the wider diabetes multidisciplinary team to provide specialist diabetes support, advice and education for patients with type 1 and type 2 diabetes as well as other healthcare professionals. We are seeking a nurse who has a positive approach and is committed to making a difference. You will need to demonstrate a keenness to work in the field of diabetes, effective communication and teamwork. Experience of supporting patients and carers with diabetes management would be an advantage but not essential as training and development in role can be provided. The role will include working within a variety of clinical settings: the inpatients diabetes nurses assessing and reviewing people with diabetes who are inpatients as well as working within the outpatient department in diabetes clinic and some community based diabetes clinics
Main duties of the job
The post holder will provide on-going specialist education and skills training for patients, carers and staff around management of diabetes as well as promote self-management strategies and provide opportunities for health promotion.
The post holder will contribute to the implementation of evidence based practice in line with national and local diabetes strategies and guidelines and will be given the opportunity to support service development or audit projects within the department.
The role is predominantly based in the diabetes centre with outpatient clinics and inpatient reviews on the wards and will also include some community clinics.
There is scope to develop knowledge in new advances in the care of those with Diabetes such as blood glucose analysis and insulin pumps and linking in with the wider Diabetes MDT such as medical team and Diabetes Specialist Nurses.
The post holder will carry responsibility for either a defined case load or offering specialist service advice, this will include setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care, management and education.
The post holder will contribute to achieving the objectives of the clinical service group/specialty corporate objectives and work within the clinical governance framework, through the core dimensions of the role, clinical practice, leadership, acting as the patient advocate and through audit and education.

Providing individualised education to patients and carers to support diabetes management and promoting healthy lifestyle.
