Community Learning Disabilities Nurse - Band 5

We are currently seeking a passionate and motivated Learning Disabilities Nurseto join our community team. This opportunity is ideal for a newly qualified nurselooking to begin their career in a supportive environment, or for an experienced practitionerseeking to further develop their skills in community-based learning disabilities nursing.

You will be joining a well-established and experienced team of Learning Disabilities Nurses who are committed to delivering high-quality, person-centred care. Comprehensive support and mentorship will be provided, along with access to funding for continuing professional developmentto support your growth and career progression.

Following a structured induction period, you will manage a caseload of individuals with a range of healthcare needs. Your role will involve working directly with service users, as well as collaborating with families, carers, and social care professionals to ensure holistic and effective care planning. Key areas of focus will include:

  • Supporting mental and physical health
  • Managing epilepsy and memory-related conditions
  • Promoting wellbeing through personalised care plans

Main duties of the job

  • To supervise and assess student nurses on placement with the team.
