We are a specialist Physical Health Team that has recently expanded within Adult Community Mental Health Services in Black Country Healthcare NHS Foundation Trust
The teams focus is to improve the physical health outcomes for our patients with direction and standards issued by the Care Quality Commission, NHS England and NICE guidelines.
You will undertake cardio-metabolic monitoring in Out-patient Clinics and complete some home visits when needed.
Main duties of the job
The role of the STR Worker is to support in Out-patient Clinics and complete some home visits to undertake annual cardio-metabolic physical health checks. You will monitor physical observations; BP, weight/height/BMI, complete ECG's and take bloods (all training will be provided). You will also support senior clinicians within the one-stop Clozapine Clinic.
We are seeking an enthusiastic and motivated individual with experience of working with adults who have severe and enduring mental health difficulties. Applicants must have excellent communication skills, good IT skills and a genuine interest in improving the physical health for patients. You should be able to form good working relationships with the multi-disciplinary team to deliver a safe, high quality and effective service. You will be supported with supervision from senior members of the team.
