Job summary

An exciting opportunity has arisen for a Registered Mental Health Nurse to work in partnership with Dudley CVS's Voluntary Sector Social Prescribing Team, enhancing the existing role of the HIUS link workers/social prescribers.

Individuals who are high users of services have a combination of complex needs and an overrepresentation of mental health difficulties, alcohol and drug related use, complex physical health disorders and a strong association with higher socio-economic deprivation, which leads to an increase in attending the Emergency Department at Russel Hall Hospital and accessing Primary Care Services. These individuals are often socially isolated and can face exclusion, for various reasons; therefore, finding it difficult to engage in the traditional planned care or follow up.

The post holder will work collaboratively with the HIU service in Dudley allowing autonomous professional responsibility for biopsychosocial assessment and offer specific mental health interventions to add value to the existing non-clinical social prescribing interventions, to identify unmet care and social needs and improve access to services.

Main duties of the job

The post holder will develop links with mental health colleagues to access required care, support and intervention with local mental health services and attend professionals meetings as required.

The post holder will also provide reflective practice, supervision to the HIUS link workers/social prescribers within the team.

You will require experience of working with individuals with mental health difficulties, an ability to undertake a biopsychosocial assessment and formulation of risks. Excellent communication and interpersonal skills and an ability to work in collaboration with all partner agencies.

Essential skills in recording clinical documentation onto the BCHFT electronic patient records and CVS CRM system and also supporting the line manager with data analysis

You will be based at an office in Brierley Hill working Monday to Friday 9-5pm with some flexibility to work unsocial hours to meet the individuals needs if required. The role will include travel across the Dudley borough and individuals will be seen in a variety of settings, including their home, in a primary care setting and at times within the Acute Hospital. Therefore, you must hold a car/motorcycle licence and be able to mobilise within the community locality and to meet the needs of the service.

To engage with and provide intensive support for individuals on the identified caseload aged 17+ with complex needs.

The post holder will be an experienced Mental Health Nurse allowing autonomous professional responsibility for assessment, intervention, supervision and discharge.

The intervention provided will improve individuals access to required care, support and intervention within local mental health systems. This provision of this is based on the known over representation of mental health and emotional needs in this cohort.

The post holder will work collaboratively with both urgent and planned care mental health services, substance misuse services, 111, West Midlands Ambulance Service, West Midlands Police, the Acute Trust and Voluntary Sector Organisations.

To provide in reach if an individual on the caseload is admitted to the Acute Hospital or Mental Health Hospital to ensure continued shared understanding and joined up working.

To work within defined competencies and without direct supervision to provide a flexible range of emotional, social and practical support to a rolling caseload primarily within the community and their own homes using a truly personalised approach.

To work closely with individuals to help navigate complex health and social care systems based on identified needs.

To complete agreed trust datasets for each patient including core assessment documents and risk assessment. The post holder will complete patient reported outcome measures and the start and end of intervention.

To participate in providing training and learning opportunities with the existing HIU team, key stakeholders, voluntary sector health and social care providers

Identify any risk the individual may pose to themselves and others, using appropriate escalation along with safeguarding channels and dynamic risk assessment.
