Job summary
An exciting opportunity has arisen to join our established, passionate and driven team on Meadow ward. We are seeking to appoint enthusiastic and motivated individuals who have a particular interest in working with older adults.
The successful candidate will contribute to the service already being delivered and will participate in the development and delivery of activities to meet the individual needs of service users, providing social and recreational/diversional activities to the in-patient services.

Main duties of the job
? To assist in basic care and assessment of the patient as support to, andunder the supervision of, a registered nurse
? To follow individualised care and therapeutic programmes for patients under the instruction of the nursing staff.
? To provide support to patients with their personal care.
? To be aware of the patients clinical condition and life storyto ensure the care and comfort of clients.
? To promote communications and act as an advocate for patients who have experienced difficulty in communicating.
? By communicating any observations of patients behaviours to the nurse-in-charge during periods of duty.
? To work with all members of the multi-disciplinary team.
? Work in collaboration with the patient, involving them at all times with their care.
? To ensure the continuation of good working relationships between staff and patients within the unit.
? To support the qualified staff in the admission and discharge procedure.
To support staff in promoting recreation/leisure amongst patients.
? To contribute to the promotion of patients education and well-being.
? To monitor and encourage and record patients in their consumption of food and drink.
? To assist in the maintenance of a safe environment.
? To participate in the observation and engagement of patients at risk.? To promote and maintain beneficial relationships between relatives/patients and carers in liaison with other appropriate agenciesTo support with escorts to hospital and community

Meadow ward is mixed function ( functional and organic) with Male and female patients. The ward environment is, modern, clean and spacious which lends itself to being a successful therapeutic environment, for both functional and organic patients.
The team comprises of a large MDT which includes; Nursing, activity coordinator, medics, OT, Physio, SALT, Psychology and dietitians. You will also have the opportunity to work alongside senior staff nurses with specialisms, Matrons and clinical lead.
The successful candidate will have access to specialist training, such as dementia, and physical health training. The trust are offering a new variety of training and progression for HCSW. As well as at ward level, by a variety of experienced staff including, clinical leads, matrons.
