An opportunity has arisen for a full time band 5 CPN to join our Wellbeing service in Wolverhampton.The post-holder will play a key role in the case management and assessment of clients in the mental health clusters 4-7 and 11.
Main duties of the job
The post holder will help provide a range of psycho-social interventions to working age adults, with mental health problems ranging from mild to severe.
We are looking for a CPN with an interest in developing their skills in therapeutically orientated clinical work and service development. The post-holder will develop and deliver packages of care based on the recovery model of mental health. The post holder will also undertake a duty worker role for the Wellbeing Service.

Job responsibilities
The post holder will have responsibility for a client caseload and will be proactive in the assessment of client need and the development and implementation of packages of care.
To provide care appropriate to their level of training as specified within the competencies required and in accordance with the NMC Code of Professional Conduct.
To participate in formal/ structured supervision with the senior nurse in accordance with BCHFT Trust policy.
To develop and maintain packages of care that are formulated to meet the mental health needs of clients and place the rights and the dignity of the individual at the forefront of practice.
To provide, with minimal supervision, high standards of professional care through mental health/ psychiatric nursing. This may be in the clients own home and/ or a variety of other community settings.
To accept Wellbeing referrals, with supervision from senior nurse, ensuring in all cases requiring nursing intervention, there is a responsible clinician identified.
To participate within a duty system
To contribute to the assessment process and where necessary take on the role of keyworker for clients.
