An exciting opportunity has arisen within the Memory Assessment Service in
Wolverhampton based at Brooklands Parade Health Centre. The service operates 09:00hrs to
17:00hrs Monday to Friday. We are currently looking to recruit an experienced
RMNs/RGN with previous experience working with older adults to work with the Memory Assessment Service. The posts are fixed for 12 months period however there is a likelihood of these posts to be reviewed and converted into permanent post thereafter as part of the Service Transformation. The successful applicant will be responsible for the assessment and planning of care for individuals over the age of 65 who may have dementia or memory difficulties.
We are looking for an enthusiastic and committed individual who will be part of a very committed and hardworking team comprising of RMNs, Psychiatrist Consultants, clinical psychologist, support worker and working closely with Occupational therapists and admiral nurses in older adults division.

Main duties of the job
- To carry a defined caseload of patients- manage and prioritise the caseload work.
- To be responsible for specialist nursing assessments of people with memory problems and of their carers [including risk assessment].
- To offer practical support and guidance to carers.
- Participate in multi- professional team meetings and joint working arrangements with other statutory and voluntary agencies.
- To follow and implement the Trust's Policy on 'Safeguarding'.
- To develop and implement programmes of care and treatment.
- To utilize effective time management skills
To set clinical standards and develop Integrated Care Pathways and protocols within the Memory Service based on Evidence based practice and current research and developments

Will be working providing care and support for a defined group of patients (caseload) who have been accepted for an assessment by the Memory Assessment Service.
- Under the supervision of the Team Manager, arrange and organise appointments for service users of the Memory Assessment Service, and conduct assessments and reviews of treatment with cognitive enhancing medication, cognitive functioning and other relevant service user and carer related issues, both in the home, clinic and residential care settings. Both verbal and written liaison to the appropriate person or agency will be required.
- The post holder will be responsible for the smooth, overall day-day running of the service, ensuring policies and clinical procedures are relevant to working practices, maintenance of a safe environment, supervision, mentorship and participation in appraisal process
- Work in partnership with the multi-disciplinary team and contribute to the continuing development of the service.
- To facilitate access to relevant services in the statutory and non-statutory sector.
- To participate in any duty system
