Main duties of the job

  • To provide individual or group interventions to service users and/or carers in accordance with their care plan.
  • To make relevant service user/carer observations and to share with relevant members of CRHTT/MDT and other agencies.
  • To conduct solo and joint contacts with service users in the community.
  • Support service users with medication management.
  • Positively support independent living within the community.
  • Provide information on health promotion.
  • Help service users gain access to community resources and support with access to benefits and welfare rights.
  • Develop and maintain therapeutic relationship within appropriate and transparent boundaries.
