An exciting opportunity has arisen for a fixed term Health and Wellbeing Coordinator to join our Late Effects team. This post is being offered on a 12 month basis. The successful candidate will work alongside our Late Effects MDT to ensure our Children and Young People can access appropriate signposting and support following treatment for Cancer.
The role will support the MDT in administration of clinics and also in promoting health related information and support to our children, young people, and families. The post holder will make meaningful relationships with children and families who require compassion and expert advice throughout their journey, ensuring we show our Trust values to every patient - Ambitious, Brave, Compassionate.

Main duties of the job
The role will support the MDT with clinic administration and provide practical support to children, young people, and their families, helping to promote healthy lifestyles. The post holder will make meaningful relationships with children and families who require compassion and expert advice throughout their journey, ensuring we show our Trust values to every patient - Ambitious, Brave, Compassionate.
The successful candidate will support the Late Effects MDT to ensure patient consultations are organised effectively and efficiently so the patient is seen without clinical risk, within agreed timelines and Trust standards, including the safeguarding policy. Under the guidance of a registered practitioner, the successful candidate will ensure the child, young person (CYP) and families are aware of their long term follow up pathway and empowered with information about potential/actual late effects and how best to manage them.
