Job summary

We work with young people experiencing emotional dysregulation and have high levels of risk.

We have small caseloads so that we have time to offer both short and longer term intensive interventions to enhance the support offered from the CORE Teams, and also time to think and reflect to support both the young people, their families, and the system.

You will be involved in the shorter term stabilisation work . You will be supported by the Clinical Nurse Specialist and the Lead Nurse.

You will manage a small case load and co work with members of the team, offering supervision / line management to the clinical support worker. You will be part of an MDT who work together to support you to support the young person using Trauma models and Trauma informed care to offer support.

We have a hybrid model of working between the home and office in at Finch Road Primary Care Centre in Lozells Birmingham, and most client contact is face to face across the city.

Main duties of the job

Assessment and collaborative care planning with the young person to empower young people to reach their goals.

To implement and monitor care with the young person using specialist nursing skills based on trauma informed care and trauma-based psycho education and skills.

Liaison with both internal and external services to support the young persons network.

Build trusting and effective therapeutic relationships with young people to help support distress and foster hope.

Management of risk and empowerment of young people to be active within this process.

Provides day-to-day supervision and/or clinical supervision to others.

Provides Line management support to clinical support workers.
