Job summary

The Bed Management Team is the single point of access for all referrals for acute inpatient admission. The post holder will co-ordinate and manage patient flow across Adult Acute Inpatient Services (including PICU) ensuring the best quality of care for each patient. The post holder will work alongside the Lead and Senior Management Team to ensure effective patient flow and a quality lead service. Coordinating all bed requests and enable streamlined capacity management for all. Provide support to ensure all admissions are purposeful face to face gatekeeping happens with a consistent threshold for admission overseen by the bed management team. Clinical System recording Management and leadership of daily communications including leading bed management calls; ensure bed sitrep and urgent care sitrep is circulated. Oversight of all patients admitted as well as attendance at all bed management meetings. Central point for Out of Area Placements (OAP), Coordinate and support planning of, outcome of and change in circumstance for expected DTAs, bed requests, planned MHAA and police S136 calls. Providing a consistent approach to all WL patients in regards to patient flow systems/processes and enable a seamless bed management/discharge coordination pathway. Support with potential breaches to reduce escalation to out of hours on call manager and minimise bed escalations from A&E.

Main duties of the job

To ensure all potential admissions will first be seen and assessed by the CRHT/LP/EI Team

  • To clinically validate that admission is most clinically appropriate option through trusted assessment / FTB consultant approval and use of admission support tool. To provide clinical challenge where admission is not indicated best option for the patient.
  • To ensure all paperwork for admissions is completed and Emailed to relevant hospital, to include, assessment, care plan, risk assessment.
  • To ensure copies of mental health act paper work is copied to care notes.
  • To ensure where required funding forms for the out of area beds, are escalated and signed and forwarded to relevant hospitals.
  • For the use of any out of area bed a datix incident report should be completed by the bed manager.
  • To ensure where there is high demand for beds and no availability to be responsible for allocation of bed in accordance with the Trust admission policy. To work creatively with BSOL provider collaborative to ensure that all patients are clinically prioritised.
  • To ensure area reports are timely and the updated information is recorded daily and reported on the situational report.
  • When required due to levels of admissions and unavailability of beds to instigate the triage system (clinical prioritisation) as per Bed
  • To participate/lead on safeguarding issues as appropriate
  • To liaise with ward staff and support requirements for admission and discharge

  • Registered Nurse with a current professional qualification (RMN, RSCN, RNLD, Post registration training in a treatment modality to diploma/degree standard as a minimum such as CBT.

Desirable

  • Leadership qualification/ Management Training

  • Extensive experience in Mental Health work with young adults and acute pathways.
