Job summary

We are looking for a new colleague to join our friendly, supportive department. This substantive post will be primarily based at the Princess Royal Hospital in Haywards Heath. This job involves the specialty assessment and management of older people with frailty through the medical take including ongoing responsibility for patients with frailty admitted on PRH AMU, leadership of the acute frailty team, specialist advice to primary care and community colleagues, admission avoidance activities and advance care planning. There will be opportunities to contribute to the development of our frailty SDEC@PRH and community geriatrics services.

Main duties of the job

The clinical duties of the post include the following:

Day to day care of patients and leadership of the frailty team on PRH AMU and frailty patients in ED. This includes participation in daily board rounds, daily ward roundsrounds, and participation in ward daily management system meetings. Responsibilities will include proactive identification of patients in ED providing comprehensive geriatric assessments, consideringassessments, considering existing admission alternative pathways including utilisation of general virtual ward and working with colleagues to develop pathways alternatives to ED attendance and admission

At PRH there is an oncall consultant This post will provide a 1 in 8 frailty weekend at PRH 8-4 saturdaySaturday and sundaySunday, working collaboratively with the GIM consultant oncallon call and a 1 in 8 overnight GIM on call. This will include the post take review of older patients with frailty admitted in the previous 24 hours.

Alongside colleagues develop Frailty SDEC@PRH and explore community geriatrics opportunities in the local area

Cross cover for leave with other acute frailty and ward consultants at PRHPRH.

Opportunity to rotate to spend short periods on C8 ward at RSCH.

Time off in Lieu will be provided for an bank holiday working

  • oBasic medical degree
  • oMRCP(UK) or its recognised equivalent.
  • oFull and specialist registration (with a licence to practise) with the GMC or be eligible for registration within six months of the interview. o If you trained in the UK, you must also have a Certificate of Completion of Training (CCT) in Geriatric Medicine and General Internal Medicine or be within six months of receiving it by the interview date. o If you trained outside the UK, you must provide evidence that your qualifications are equivalent to the UK CCT.
