Job summary

We would like to take this opportunity to identify a suitable candidate to join the Thrombosis and Anticoagulation nurse led team at St Richards and Worthing Hospital in a Band 6 CNS role.

We are therefore seeking applications from candidates who have the appropriate clinical experience, leadership potential, and professional competencies required to fulfil the responsibilities of this role.

Anticoagulation nurse led services across UHSx are in a process of merging with a vision to establish a symbiotic anticoagulation service for all our clients.

The successful candidate will work closely within the Thrombosis and Anticoagulation team across both sites supporting the Deputy Thrombosis and Anticoagulation Nurse (B7) based at SRH & WG.

Main duties of the job

The role of the Thrombosis and Anticoagulation Clinical Nurse Specialist (CNS) is a highly specialised role that will involve providing expert care, advice, guidance and professional judgement within the remit of Venous Thromboembolism (VTE) and Anticoagulation for our service users, Trust professionals and external care stakeholders.

As a CNS you will be working as part of a MDT to manage the care of our anticoagulation and thrombosis patients within this geographical area.

Working closely within the VTE/Anticoagulation Nurse team with the Haematology consultants & anticoagulation Pharmacist, the CNS provides an anticoagulation dosing and advice service, for patients on Vitamin K Antagonists and direct oral anticoagulants.

Our client group includes a variety of patients with pro-thrombotic conditions requiring safe & effective management of their anticoagulation. This care is delivered across both hospital sites.

To assist with the in- patient management of the complex patients providing specialist knowledge and ensuring transition of anticoagulation management plans between primary and secondary care

As a thrombosis and anticoagulation nurse specialist, you will be expected to manage complex anticoagulation patient cases and take part in the Anticoagulation MDT meeting to ensure sound governance within this service.

There is no specific caseload of patients to manage, but there shall be shared responsibility for the cohort of patients attending the service

You will be responsible for autonomously running an anticoagulation clinic for your client group under the overarching supervision of your senior lead specialist nurses.

An important aspect of this role is to liaise with the community health professionals regarding the care of your patients and to provide education on venous thromboembolism and anticoagulation for patients and other health care practitioners
