Senior Thrombosis & Anticoagulation Clinical Assistant

Job summary

The role provider will have responsibility for the organisation of anticoagulation clinics and the day to day smooth running of the service, receiving referrals and dealing with complex anticoagulation queries. In your role working face to face with our service users, you will be the initial contact point representing the Anticoagulation Team for our service users and stake holders.

Under the leadership and guidance of the Thrombosis and Anticoagulation Administration Manager, you will be able to use your own judgement to determine when advice from senior colleagues is required.

To follow all department protocols in dealings with both colleagues and clients, contributing and implementing changes when required

Raise concern and escalate appropriately where unsatisfactory practice is identified

The post holder must be able to work autonomously and also in conjunction with the administrative team, clinical nurse specialists, Consultants and internal and external stakeholder

Main duties of the job

Act as a point of contact for patients /carers', dealing with enquires from service users and other stakeholders.

Attend and run point of care clinics for our patients.

Liaise between laboratory staff and service users in a polite and helpful manner.

Attend meetings including multi-disciplinary meetings ensuring effective communication and dissemination of relevant information.

Provide anticoagulation referral reviews, add referrals onto DAWN database and CAREFLOW. Book clinic appointments.

Identify and problem solve on-going service issues and report to Admin Manager.

Close working with the Admin Manager to ensure constantly updated system reviews, patient management and best practice.Answer telephone line including managing the answerphone system, dealing with complex anticoag queries. Passing on advice provided by the CNS when required. .

Use the 4s DAWN Anticoagulation Dosing System and enter patient data.

Upload relevant documentation into DAWN and to develop a paperless environment.

Perform quality control testing (NEQAS) for coaguchek equipment.

To work as part of a team and liaise with Medical Laboratory Assistants, Biomedical Scientists within the Haematology and Pathology Departments.

To work alongside Trust colleagues in order to contribute to the Trust vision We Care and to contribute fully towards UHSussex NHS Trust corporate objectives, divisional and departmental delivery plans. Demonstrate respect, dignity and privacy of all staff and patients and act with kindness and courtesy.

Attend regular staff meetings including multi-disciplinary meetings to ensure effective communication and dissemination of relevant information.

To provide fast anticoagulation referral reviews, adding referrals to the DAWN database and CAREFLOW. Book clinic appointments where appropriate.

To identify and problem solve on-going service issues and report to the Administration Manager and/or Clinical Nurse Specialists as appropriate.

Liaise closely with the Administration Manager, Service Lead and Clinical Nurse specialist team highlighting any issues in a timely manner

Ensure all clinical responsibilities are conducted according to Trust policies and guidelines regarding infection control and prevention.

Contribute to the provision of a safe and therapeutic environment for all staff, patients, relatives, friends and hospital visitors.

Answer patient telephone line including managing the answerphone system, dealing with complex anticoagulation queries, rebooking appointments as appropriate. Passing on advice provided by the Clinical Nurse Specialist when required.

Book patient clinic appointments using CAREFLOW
