Job summary

Support the administration of the pathways for patients diagnosed with cardiac conditions such as heart failure, Aortopathy and inherited cardiac conditions.

Triaging patient emails and phone calls and dealing with administration enquiries.

Ensuring all patient and clinical information is available to make informed treatment decisions.

Collation of test results for MDT discussions, and for clinic appointments.

Co-ordinating and booking nurse clinic appointments and telemedicine appointments.

Recording outcomes to reflect activity accurately.

Support the processes required for effective MDT meetings including record keeping and onward referral.

Job responsibilities

Main Duties and Responsibilities

Communication

To monitor the specialist nurse shared email inboxes, dealing with administrative queries and highlighting anything urgent.

Liaise with the waiting list co-ordinator for enquiries regarding wait times or changes to status on the waiting list (i.e. cancellations, postponing cases, upgrading cases to urgent) and keeping the nurse specialists updated.

To communicate daily with a wide range of staff within the NHS and private health care organisations. The post holder must be able to communicate with the cardiac bookings team, General Managers, Clinical Directors, Consultants, Service Managers, Ward Clerks, community teams.

To ensure the clinical team are advised of any patient who requires clinical support.

To ensure all information and contact with patients is recorded accurately and disseminated to the appropriate team member.

Service Delivery and Improvement

To work with the specialist nurses to improve data quality.

To work with the specialist nurses to produce audits associated with the working of the MDT.

To work with the specialist nurses to roll out patient surveys and to record and collate the information.

To support clinical audit by providing data entry onto clinical systems.

To work with the specialist nurses to improve the service and to ensure that all services are monitored.

To process map problem areas with the aim to reduce waiting times, organising meetings to discuss as required.

To ensure operational meetings are supported from an administrative perspective.

People Management and Development

To assist in the process of induction and training for new clerical staff.

Patient Care Delivery

To answer calls to the specialist nursing teams, triaging and taking all relevant information so that it can be dealt with effectively. Deal with administrative enquiries independently.

To contact patients, where appropriate, to explain the process for individual treatment pathways.
