Job summary
This role is eligible for visa sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route however sponsorship can only be provided where applicants meet the specific requirements set by UKVI, including the relevant skill and salary thresholds (£25000), and any role-specific criteria.
The Discharge Team is based on both sites ,The Royal Sussex County Hospital in Brighton and Princess Royal Hospital (PRH) in Hayward Heath. RSCH Discharge Lounge is looking for 1 full time 37.5 hours and 1 part time 22.5 hours Band 5 Registered Nurses to join our friendly team. The operational hours will be from 0800 - 20:00 hours currently Monday to Friday.
The Discharge Lounge helps to promote patient flow throughout the hospital site and supports our emergency and elective demand. The Discharge Lounge is available for patients to wait for transport or medications to be discharged with, therefore creating available acute beds for patients being admitted to hospital.
We are looking for proactive, creative Registered Nurses, who are good communicators, can work collaboratively with the wards, promotingearly use of the Discharge Lounge in the day to provide capacity for the demand on our services, and creating a pleasant and positive experiencefor the patients leaving our care.The successful candidate can expect to work within a supportive team and will receive support with training and development.

Main duties of the job

Collection and care of patients whilst in the DCL, discharging patients safely, booking transport, communicating with on going community providers and NOK where required.

- To follow the trusts discharge policy and processes
- Act as patient /carer advocate at all times
- To work unsupervised, providing nursing care to patients attending the department, referring to other health care professionals as necessary.
- To prioritise care and delegate workload and lounge activities appropriately.
- To effectively and professionally deal with emergency situations.
- To ensure safe practice with regard to moving and handling, sharps and infection control etc.
- To check TTOs prior to discharge, using the Pharmacy Tracking system. Ensuring they are correctly dispensed with any necessary follow-up arrangements in place. To check the Electronic Prescribing of Medication and Administration (EPMA)
- Identify and report clinical and non-clinical incidents, taking appropriate action as necessary. Raising Datix when required.
- Update CareFlow system to contribute to an accurate and timely bed state.
- To ensure that confidentiality of patient information is maintained and that written documentation is complete and up to date.
