Introducing our new Same Day Emergency Care (SDEC) and Acute Assessment Unit (AAU). We are thrilled to announce the opening our new purpose built SDEC unit in April 2025, alongside the rebranding of our Emergency Floor as an AAU.
These units are designed to enhance patient care and streamline emergency services. Their function is to provide quick, efficient and effective care to patients in urgent need of medical attention.
Same Day Emergency Care: Our SDEC allows patients to be assessed, diagnosed and treated all within the same day, minimizing hospital admissions and ensuring fast, high quality care.
Acute Assessment Unit: currently known as the Emergency Floor, the AAU offers an immediate and thorough evaluation of acutely ill patients, allowing us to quickly identify needs and create an individual plan of care.

As a member of the SDEC/AAU team, you'll have the opportunity to rotate between both units providing essential care to patients.
Main duties of the job
The Band 5 registered Nurse in the SDEC and AAU is responsible for delivering high quality, timely and patient-centred care to individuals with acute medical needs.
The role requires clinical expertise, quick decision making skills and strong communication, as well as collaboration with multi-disciplinary teams to ensure patients' receive appropriate assessments, treatments and care in a fast-paced setting.
The SDEC will have 16 care spaces and a large airy waiting area. We will provide same day emergency care between the hours of 07:00 - 22:00 seven days a week.
We are able to offer in-house training and external courses to our nurses, supported by our practice development team and the introduction of the Acute Nursing Competency Framework.

Main Duties and Responsibilities.
Patient Assessment
Conduct thorough, timely assessments for patients on arrival, gathering vital signs, medical history and other relevant clinical information.
perform initial triage to prioritise care based on patient needs, working with the team to quickly address those with urgent requirements.
Use clinical judgement to identify changes in a patients' condition and escalate care when necessary, ensuring swift intervention.
Developing and Implementing Care Plans
Develop individualised care plans based on assessment findings, following evidence based practice and Trust protocols.
Work collaboratively with the MDT to co-ordinate careplans, including diagnostics, treatments and discharge/transfer decisions.
regularly review and adjust care plans as needed to ensure they align with patients' changing conditions and clinical needs.
Providing Direct Patient Care
Administer medications and carry out other clinical interventions as prescribed.
Monitor and document responses to treatments, communicating any concerns or changes in the patients condition to the appropriate team member.
Support patients in managing their symptoms, providing information and reassurance to promote comfort and reduce anxiety.
Maintain accurate, up to date patient records, documenting assessments, care plans, treatments and any observations or interactions
Adhere to strict infection control protocols.
Same Day Discharge and Admission
Work efficiently to support same day discharge by ensuring patients receive appropriate care and are fit to go home with clear instructions.
Facilitate admissions for those requiring further care, ensuring a seamless handover.
Provide discharge education and follow up instructions. Address questions or concerns to ensure patients and relatives understand the next step in their care.
