Job summary

We are looking to appoint our next Bariatric Senior Clinic Fellow to work in our unit for 12 months from 2 October 2024, we reserve the right to also appoint for the subsequent 12 months if two appointable candidates are identified at interview.

This highly sought after fellowship is based at St Richard's hospital in Chichester, West Sussex. It is approved by the Royal College of Surgeons of England and aimed primarily at post-CCT trainees with a career interest in Bariatric Surgery.

The hospital has one of the longest established and busiest Bariatric Surgery units in the country. It has been recognised by IFSO as a Centre of Excellence in Bariatric and Metabolic Surgery.

The post aims to offer exposure to approximately 400 bariatric procedures per year, including primary, revisional and emergency cases, working towards independent practice by the end of the fellowship. The Fellow will also be involved with the weekly multi-disciplinary new patient clinic, occasional follow up clinics and endoscopy sessions (if the fellow has pre-existing JAG accreditation). The Fellow will also be expected to be actively involved in on-going research projects and will have the opportunity to conduct new studies as part of our growing research portfolio.

The fellowship is highly regarded for its standard of training with all fellows over the last decade securing consultant positions in their chosen field.

Main duties of the job

The fellow is an integral part of our comprehensive Bariatric service providing care to new, elective, follow up and emergency patients. They should have leadership and management skills that will mean that they can help co-ordinate the care of emergency tertiary referrals from hospitals across Kent, Surrey and Sussex (and sometimes beyond).

The fellowship offers exposure to around 400 Bariatric operations per year. The fellow would be expected to perform a major part of well over 50% of these procedures. These include primary (predominantly sleeve gastrectomy, Roux-en-Y gastric bypass and one anastomosis gastric bypass), revisional and emergency cases. As the fellowship progresses, there will be the opportunity to develop independent practice and help train the registrars in the department. Elective operating time is protected, with no on call commitment during elective hours.

There are opportunities to develop endoscopic skills, including interventions for bariatric complications.

Attendance at the multidisciplinary one stop new patient clinic each Friday is another key role of the post. There are also opportunities to attend follow up appointments, including a specialist gastric band management clinic that is run by our specialist nurses and surgical care practitioner.

Additionally, there is a1 in 8 out of hours (evening/nights and weekends), non-resident on-call commitment at registrar level for General Surgery.

This is a 12 month fixed term post aimed at post-CCT Upper GI specialist surgeons with a career interest in Bariatric surgery and aims to prepare them for independent practice. It can be made available as full or part time post. It is overseen by the Royal College of Surgeons of England as part of their Senior Clinical Fellowship scheme.

St Richards Bariatric Unit provides bariatric surgery services for the West Sussex, East Sussex, Kent and parts of Hampshire. It has been recognised by IFSO as an International Centre of Excellence in Bariatric and Metabolic Surgery. It is one of the longest established and busiest bariatric services in the UK. The service is geared towards treating approximately 400 cases per year. This includes primary, revision and emergency Bariatric surgery, the fellow would be expected to have a major role in well over 50% of these cases.

Then department is a friendly and supportive place to work, with a very close knit extended surgical team across the entirety of General Surgery. Specifically, the Bariatric service is supported by a dedicated specialist Bariatric physician, a team of specialist anaesthetists, 2 specialist nurses, a surgical care practitioner, a team of supporting dietitians, a recently appointed psychologist and a dedicated administrative team.

Outpatient multi-disciplinary new patient and follow up clinics are conducted in the specialist Bariatric clinic. All new patients attend a one-stop multi-disciplinary bariatric clinic where they are assessed by a specialist physician, surgeon, anaesthetist, dietitian and specialist nurse, having undergone psychological assessment prior to clinic. The bariatric fellow plays an active part in this clinic each week.

Most weeks there are at least 3 all day operating lists, as well as the new patient clinic, follow up clinics and, occasionally, endoscopy sessions. The Fellows time is protected to allow attendance at all Bariatric operating lists and new patient clinics. Duties also include inpatient care and the supervision of other doctors in training. Another key responsibility of the post is to help co-ordinate the care of emergency tertiary referrals to the department from the South East of England.

The Fellow will be involved in the General Surgery on call rota at middle grade level (1 in 8). However, this is an out of hours commitment (weekends, evening and nights) only and should not impact on exposure to Bariatric activity. Similarly, there is an opportunity for further exposure to all other aspects of general surgery, if desired, and to run independent operating lists, dependent on the experience and competence of the successful applicant.

The post works along the lines of a Senior Registrar post, with responsibilities supervising the more junior registrars on the Upper GI unit. Therefore, it is aimed at an Upper GI trainee with advanced laparoscopic skills (preferably including suturing) in their final year of training or, preferably, post CCT. Prior experience in Bariatric Surgery is preferred but not essential as we have successfully trained surgeons to independent practice in Bariatric Surgery with no prior experience.

We have developed strong links with the Universities of Surrey and Chichester and there is an opportunity to be heavily involved in research and audit projects. The Trust has a very supportive R&D department. Opportunities are also available in developing management, leadership and education skills during the fellowship.
