Pleural clinic/unit set-up
-
- This will provide the patients with direct access to a pleural specialist rather than the current convoluted route of presentation via A+E
- This will also aim to reduce admissions and keep these patients' management in an out-patient setting
- Bespoke new hospital including a respiratory unit will provide a 'blank canvas' for a successful candidate to lead a unit with full support and new pathways.
- Pathway for speedier diagnosis
- Earlier intervention with correct test and diagnosis.
- Opportunity for research projects
- This can either be via the British Thoracic Society or through the many national trials that are regularly opening for recruitment in pleural disease
- Delivery of teaching and training
- Confidence and competency in pleural procedures is a real challenge for trainees in the medical specialties across the board. It is often one of the more challenging competencies to attain as an IMT and also something that needs to be engulfed in the continuing training of HSTs with opportunity to keep skills up in supervised practice environment. This could be achieved in a pleural unit/clinic as organised by the fellow.
- Core member of lung cancer team with MDM working
- Sharing and discussion of cases in MDT with thoracic surgeons and oncologists for input and management strategies.
- There is aspiration that a medical thoracoscopy service would be set up which a successful candidate could help drive this forward.
Main duties of the job
- Provision of pleural clinic lists with teaching and training of junior staff. This will be 2-3 sessions per week
- Attendance at lung cancer MDM (1200-1400 Tuesday)
- Respiratory clinic (one per week with aim to support lung cancer clinical activity).
- Respiratory in-patient care
- Respiratory SDEC as a separate entity to pleural clinic with organisation of services.
The successful candidate will have ample opportunity to train in other lung cancer related procedures to provide a fully rounded exposure and training portfolio. There is flexibility to many of the commitments and clinical activity can be reviewed to meet the needs of applicant if appropriate.
There will be mentorship and clinical support provided by consultants in the respiratory department with designated educational supervision for this period with supervised training.

Lung cancer has historically been associated with poor outcomes due to the late presentations of patients with advanced and less treatable disease and also due to the complexity of the diagnostic pathway. Attempts to address this have been put in place with systems such as total lung health checks to identify patients earlier but also work on the lung diagnostic pathway is needed.
Pleural disease is a common presentation for patients attending the emergency department and quite frequently the index presentation of malignancy. Over the past decade it has been appreciated that these patients have better outcomes when they are managed by specialist team with a clearer pathway and earlier diagnosis. There has been an emphasis on specialist input into the management of pleural disease with earlier assessment and intervention by many of the professional societies (e.g. British Thoracic Society, European Respiratory Society).
Ideally a patient with pleural disease should be worked up through a rapid access pleural clinic with parallel discussions through the lung cancer MDT. This will enable faster diagnosis, streamlined pathways and better outcomes for patients.
