Job summary

An exciting opportunity has become available for a Band 6 Nurse Team Leader to join our team in the Lawson Unit. The Lawson Unit provides excellent care to people living with HIV across Brighton and the South East.

The Lawson unit provides a range of outpatient care to a diverse group of patients. The nursing team provide routine blood monitoring, health promotion and psychosocial support to our patients. We provide a range of specialist clinics for people living with HIV including, women, older people and people experiencing memory issues. Nurses are regularly involved in research and audit projects and supported to present at and attend conferences.

We have a strong multidisciplinary team which enables us to provide the best care possible. We are looking for a motivated and enthusiastic person with knowledge of HIV care to join our friendly and dynamic team. Excellent interpersonal communication skills, maintaining professional boundaries and leading by example are key expectations of this role.

Main duties of the job

Responsible for the assessment, planning and implementation of care. Assist in the interpretation of data relevant to the patient's condition and progress. Able to interpret blood results for treatment outcomes, toxicities and side effects and utilise appropriate forums for discussion and guidance.

Assist in the identification of patient care needs with the patient, their carers, senior nursing staff and the MDT. Utilise knowledge of physical and psycho-social needs of HIV patients and is able to manage appropriately and refer on as necessary.

Have the ability to assess the complex needs of patients with HIV attending the unit, including liaison with nursing and medical staff to support planning, implementing and evaluating care.

Required to co-ordinate the follow up of HIV patients in line with agreed pathways within the department.

Maintain accurate documentation to facilitate the continuity of care for patients.

Required to provide nurse-led HIV patient follow up either via telephone/email/Face2face and follow-up clinics according to local protocol and competencies.

Carry out nurse triage referring to the HIV emergency clinic, Team meetings, Sexual Health & Contraception (SHAC) service, outside agencies and follow up patients as required.

Provide education and support for patients taking antiretroviral [ARVs] medication, having a sound knowledge of short and long-term effects of ARVs including the impact of suboptimal adherence.
