This is an exciting opportunity for an experienced Learning Disability Nurse to take on a leadership role within the Complex Physical Health nursing team, as part of the Community Learning Disability Team in Greenwich.
The post holder will be part of an experienced nursing team comprising of nurses and specialist support staff, who work towards improving the physical health and wellbeing of adults with learning disabilities within the local borough.

Main duties of the job
The post holder will be expected:
- To provide visible, high calibre nursing leadership and support around the 'Complex Physical Health' (CPH) of Learning Disability within a multi-disciplinaryframework.
- The post holder will lead the development of complex physical health of Learning Disability practice, promoting the clinical and professional development of staff within the community team, to ensure they provide holistic and evidence-based care.
- The post holder will provide specialist assessment, intervention and management,working collaboratively with colleagues, service users, families and other agencies.
- The post holder will lead on providing robust health liaison to acute and primarycare to ensure our service users are able to access mainstream health services.
- To contribute to the continuing development of the ALD Service Physical Health Group.
