The post holder will work as an experienced clinician as a key member of the discharge coordination team based within the acute inpatient services (working age adult and Older peoples). The post holder will work collaboratively with the Patient Flow Hub, inpatient wards (internal and external), crisis teams, relevant community teams / services, service users and their families, working in a targeted way to achieve the best pathway for inpatients on discharge with early identification of factors that would potentially delay timely discharge and impact on provision of appropriate aftercare.
The specialist housing worker will work collaboratively with the housing department to support patients for whom accommodation is a barrier to discharge. They will also work collaboratively with Patient Flow Hub, inpatient wards (internal and external), crisis teams, relevant community teams / services, service users and their families, working in a targeted way to achieve the best pathway for inpatients to facilitate discharges and provide supports for vulnerable individuals with complex needs, such as homelessness, enduring mental health challenges, or substance misuse, to find stable accommodation and develop independence. These roles involve conducting assessments, creating person-centred support plans, making referrals to other services.
Main duties of the job
To attend morning briefing daily and other team meetings
Completion of IDT Screening Tool within 48hurs of new admission
Attend ward rounds, professionals meeting and other MDT meeting.
Complex case management and allocation of caseloads based on Broughs.
Liaising with family, and other agency and professional
To follow the Escalation process
To support and co-ordinate timely and appropriate assessment and safe discharge of people from acute inpatient beds
Provide regular, informative, and accurate bed management information
To work closely with the inpatient wards and Home Treatment Teams and be a lead contact for discharge coordination
