Mental Health Liaison - Pathfinder

Job summary

The post holder will be based within ED providing prompt screening of Mental Health needs and risk assessments to patients presenting with solely mental health related issues. The post holder will work alongside the ED triage team to identify patients where the presenting needs is solely due to their Mental health. Following a comprehensive risk assessment and screening using the UK Triage scale the post holder will determine the most appropriate care pathway. This could include admission into ED for a full mental health liaison assessment and support, or re-direction to the Assessment and Admission Avoidance Team (AAA), community mental health services, primary care or other organisations who can best meet need. The post holder will play an important role in building and enhancing the relationships between ED and mental health services and contribute to the effective triage function of the ED. The post holder will work within the principles of providing Timely care to enhance patient experience. As a senior clinical lead, the post holder will be expected to contribute to ensuring a high-quality liaison service, participate in auditing and other areas of quality assurance, as well as supporting the development of both the service and colleagues working within the team.

Main duties of the job

  • Provide clinical leadership to the mental health liaison team.
  • To ensure that effective communication is always maintained
  • To be responsible for the setting and maintain of standards of care.
  • Liaise with community and inpatient teams regarding gatekeeping of patients to offer an alternative to hospital admission
  • To maintain accurate records, collect statistical and research data and write clinical reports
  • To be fully conversant with statutory and non-statutory services in the boroughs of Greenwich/Bexley/ Bromley and to promote effective liaison and operational links as appropriate.
  • Participate in the development of Mental Health Liaison Services
  • Represent the MHLT services as required.
  • Participate in the evaluation of the service in conjunction
  • To maintain a good public image of the MHLT services with any service user, visitor or carer by professional practice and conduct however difficult the circumstances.
