Job summary

Greenwich OA CMHT is a multidisciplinary team offering clients over the age of 65yrs, bio psycho social support with their mental health and social care needs. We are looking for an experienced Mental Health Nurse to work within the Greenwich Older Adult Community Mental health team. We really enjoy the specialism of looking after Older Adults and providing individualised, person-centred care.

The post holder will be someone with a real interest in older adult mental health and a desire to learn and develop this specialist knowledge. You will be caring and compassionate and have the patient at the centre of what you do. The Greenwich Older Adult Community Mental health Team is an integrated Mental health team which comprises of Nurses, Social Workers doctors and other disciplines. The post holder is expected to carry caseload, assess and reviews both mental health and social care needs of clients. You will work with a range of partnership services including the Royal Borough of Greenwich, GPs,other services and individuals to assess and identify appropriate treatment pathways for clients referred to our team. The post holder will also be expected to triage referrals under the duty system.

We are a very supportive team and work collaboratively across the disciplines. The post holder will be supported by a well-established specialist multi-disciplinary team. There is opportunity for professional development for post holder and regular supervision and appraisal.

Main duties of the job

Responsibility for assessing individual care needs and the planning, implementation and evaluation of care plans, in line with Oxleas Foundation Trust CPA policy.

To communicate effectively within the multidisciplinary team (and with service users and their carers), on a range of issues that are often complex in their nature and in complex situations.

Participate in supervision and Professional Development Reviews and demonstrate reflective practice through the Knowledge and Skills Framework.

To maintain electronic and written clinical records ensuring confidentiality is maintained at all times.

Provide supervision, mentorship & leadership for junior staff within the CMHT & Student Nurses, as appropriate.

In line with national and local changes in the provision of integrated services, the post holder will undertake a specialist mental health role in an holistic approach to patient management in partnership with primary health care, community health services, and social care services.

To accept care co-ordinator responsibility for a designated caseload and to manage this within the CPA framework, using a person centred approach to care.

Actively engage with service users and their carers in the provision of holistic, needs-led care, which takes account of the physical, psychological, emotional, social and spiritual needs of individuals.

To be responsible for the inclusion of social care services in the careplan and for ensuring that services are purchased in line with assessed need, agreed eligibility criteria and agreed procedures.

Have an understanding of the needs and rights of carers and undertake Carers Assessments with a view to offering supportive services.

To maintain accurate RIO records, including core and risk assessments, care plans, progress notes and contacts as required.

Have a good understanding of the Mental Health Act 1983 and the Mental Capacity Act 2007. Undertake mental capacity assessments and have an understanding of their responsibilities under both Acts.

Demonstrate an understanding of Safeguarding Adults policy and procedures.

Ensure that service users and their carers are up to date in their knowledge including having a copy of their care plan, information about treatment and the role of the care co-ordinator.

Assist patients with social and practical activities, such as education, leisure and financial management and assisting in developing and maintaining personal and social networks with families, carers, and others in line with the social inclusion model of care.

Advise and assist patients in maintaining daily living skills such as personal hygiene, self-care skills, social skills, and nutritional management.

Implement evidence based practice in all aspects of clinical care, developing an ongoing awareness of significant nursing and other mental health research relevant to the care group.

Develop effective working relationships with patients, families andcarers by facilitating engagement and therapeutic co-operation throughthe use of flexible and responsive strategies.

Develop a problem solving approach to the management of service users alcohol and drug misuse needs.

Be responsible for the administration, carriage and storage of medication in accordance with relevant policies and clinical procedures. To maintain drugs records as required by policy.

To act as a resource within the CMHT, giving advice and offering assistance as appropriate [e.g. joint visits]

To participate in duty rotas where required.

Respond to the needs of people in an honest, non-judgemental and open manner, which respects the rights of individuals.
