Job summary
The Greenwich Joint Emergency Team is looking for and enthusiastic team member to be part of a dynamic team. The team aims to prevent inappropriate hospital admissions and facilitate early discharges from hospital by acting as the gateway to the Integrated Community Services within the borough of Greenwich. The team provides a multi-professional assessment of health and social care needs, recommending admission to intermediate care facilities for rehabilitation or referring to the integrated community services for rehabilitation at home when appropriate.
You need to be a confident, autonomous practitioner with high level communication skills, a flexible problem solving approach, good clinical reasoning skills, and be efficient in your time and people management. The team provides a service in clients' own homes, care homes and the local community and provides a great opportunity for joint working, joint goal setting and regular in-service training. Main duties of the job
With support and training from the Highly Specialist Band 7 OT, you will provide specialist l OT assessment and treatment to clients presenting with highly complex medical and social needs.
The service encourages continued professional development, with in-service training within the team and quarterly professional peer supervision with all Trust OT's. This post also offers the opportunity to develop supervisory skills, by being responsible for a therapy assistants and student OTs as required. You will the opportunity to work closely with other colleagues in the South East London sector .
It is essential that you will need to be able to independently travel across the borough.

*To maintain up-to-date clinical written records using trust systems and computerised database in accordance with local and professional standards; including use of local authority electronic records.
