Intensive Case Management for Psychosis (ICMp) pathway which provides care and treatment for service users who are diagnosed with schizophrenia or bi-polar disorder.
Main duties of the job
The ICMP Team in Bromley received 120 referrals from April 2018 to March 2019.
The current caseload of the Team is 461. It has fluctuated between a high of 463 in February 2019 and a low of 444 in November 2018. The average caseload over the 12 months was 450.
The London Borough of Bromley is geographically the largest London borough in the city.
It is approximately 150 square kilometres and although relatively prosperous area there is a wide variation within the community, the northeast and northwest have high levels of deprivation and disease in comparison to inner London boroughs whilst in the south the borough compares more with the rural Kent area. Bromley has a population of 320,000. The population is expected to increase to 330,361 by 2018 and 339,154 by 2023. Approximately 71% of dwellings in Bromley are in owner occupation and approximately 13% are in the private rented sector, with 14% of social rented housing supplied through Housing Associations. The latest (2014) GLA population projection estimates show that 17.34% of the population is made up of Black and minority ethnic (BME) groups; an increase from 8.4% in 2001. Black African population experiencing the greatest increase - 1.1% in 2001 to 4.7% in 2014. The ICMP referrals average about 12 a month.

Job responsibilities
- The successful candidate will participate, with the ICMP consultant in leading and delivering the ICMP team clinical care, service development and clinical quality improvement priorities. These include:
- 100% multiaxial formulation and coding
- 100% up to date core and risk assessments, formulation & care planning as a team for approximately 465 clients
- Implementation of the NICE HTAs & Guidelines for schizophrenia, BPAD, comorbid PDs, Psychosis with substance misuse with focus on user self-management.
- Implementation of new standardised assessments, reflecting our patients complexity including HCR-20, Psychopathy checklist, ASD and Asperger, AUDIT, SADQ and neurocognitive impairment assessments
- Development of and implementation of a team evidence based substance misuse assessment and care pathway incorporating new NICE guidelines
- Development of our dual diagnosis carer support group and family inclusive practice
- Maximising use of information technology including electronic care records, intranet eLearning, assistive technologies to improve concordance, deliver home based personalised care and maximise integration with the ward.
- Development of enhanced & evaluation of a GP friendly formulation proforma for regular communication, including ensuring encounter report and prescribing records
- Assist supervision of the team core trainee, enhance the physical health of our ICMP patients, through regular medication and physical health review clinic
- Taking responsibility to develop an evaluation proforma for ICMP teams and supporting our evaluation in the comparison with other services.
- Implementation of the mental capacity, DOLs and mental health legislation
- Continuing and writing up for publication of our innovative personalised care and direct payments budget care packages.
- Undertaking audit
- Opportunity for presentation at local, and national conferences
