Bexley Community Mental Health and Wellbeing Hub - Band 6 Community Mental Health Hub Nurse Practitioner
We are looking for motivated, skilled, and experienced Community Psychiatric Nurse Practitioners to join our Bexley Community Mental Health and Wellbeing Hub.
The Community Mental Health Transformation Programme has led to the development of the Community Mental Health and Wellbeing Hub in Bexley and this will be delivered by a combination of clinical staff from Oxleas, Bexley Mind, Primary Care, Bexley LA, voluntary, third party and local partners. This will enable people to receive support with managing their mental health alongside help with issues including employment, relationships, housing, finances and engaging in the local community and accessing community asserts in Bexley. The Hub receives referrals from various sources but principally primary care sources including GPs and the London Borough of Bexley. This is an exciting opportunity to be involved with the development of the service and requires a commitment to integrated working.
We are looking for community mental health nurse practitioners with exceptional organisational and communication skills. Experience and or interests in facilitating groups/workshops.
Main duties of the job
The post holders will be responsible for undertaking comprehensive telephone and face-to-face assessments of referred patients to identify their needs; they wil have to hold a caseload for a short brief -intervention period.
The post holders will be workingin a supportive and diverseteam, where there areopportunities for personal development.
The post holders will be a mental health professionals registered with NMC with significant post qualification experience, working either in a single point of access, primary care, or community setting.

The Community Mental Health Nurse Practitioners will act as an effective members of the integrated Bexley Community Mental Health and Wellbeing Hub, with responsibility for face-to-face and telephone assessments of incoming referrals from various sources within the borough of Bexley but principally referrals from GP practices and social services teams in the borough.

To enable Bexleycare to deliver a consistent and responsive Community Mental Health Wellbeing Hub in conjunction with the ethos of the NHS Long Term Plan Community Mental Health Transformation Programme. To develop, appraise and improve the community mental health hub, working closely with all stakeholders from Bexleycare and other stakeholders to provide a robust service to people who are experiencing psychiatric and or psychosocial mental health problems in Bexley.

The post holder will deliver and or advise on the delivery of high-quality safe nursing care, ensuring consistently high standards of practice and clinical excellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct. Bexley Community Mental Health and Wellbeing Hub is a primary care level service in the Borough of Bexley. The post sits within a multi-disciplinary framework and provides assessments (telephone, virtual and face to face) and brief interventions. The team liaises and gets referrals from several sources including GPs, Bexley Local Authority and MIND.
As a Community Mental Health Hub Nurse you will promote and develop clinical leadership and clinical practice and be a positive role model to junior members of the team through the practice of evidence-based care. You will be expected to support professional development through clinical appraisal and supervision and contribute to the ongoing educational programme for staff, fostering a positive learning environment. As a senior nurse working within the Hub you are required to encourage and foster collaborative and cross organisational working.
