The Source - Nurse Led Unit
We are looking for an enthusiastic and driven individual who will support the development of the Source to meet the needs of the local population ensuring the highest possible standards of clinical practice. The post holder will act as an expert clinical resource facilitating high qualify evidence base practise.
The post holder will be practising as a qualified ACP and this will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice for both adults and children
Main duties of the job
- The post holder will have undertaken post-graduate training in Advanced Clinical Practice, while having consolidated your advanced-level knowledge and skills within your scope of practice.
- You will have gained a high-level of training and practice-based experience through the four pillars of advanced practice (Clinical Practice, Leadership, Education and Research).
- The Advanced Clinical Practitioner practices independently as an expert Advanced Practitioner who is responsible and accountable for the management of patients attending the Source.
- Leads in providing comprehensive clinical assessment and diagnosis for the delivery of effective treatment for patients presenting with undifferentiated and undiagnosed primary/urgent health care problems.
- The post holder will deliver, and lead on the delivery of, high quality safe and contemporary care, ensuring consistently high standards of practice and clinical excellence, including promoting population health, improving personal outcomes, contributing towards service evaluation, while leading and developing research and quality improvement initiatives.
- The post holder is expected to place the well-being and safety of service-users/patients above all other considerations, with responsibility to recognise and work within the limits of your professional competence.

Management responsibilities
- To undertake delegated operational management tasks and deputise on behalf of the Manager and Service lead.
- Focus on advanced clinical practice in frailty with the intention to develop and take forward in your ACP role, also taking into consideration the needs of the service.
- To continually assess the needs of the service and work in collaboration with team and service managers to monitor service needs and ways to improve the service. Assist in the development and monitoring the quality of the Joint Emergency Team/ Urgent Community service and contribute to the development of quality improvement measures, including the full audit process.
