Advanced Practitioner - ICMP, Band 7 Permanent Full Time: 37.5 hours per week

An exciting opportunity has arisen in the Greenwich Intensive Case Management in Psychosis (ICMP) service - the role of Advanced Practitioner.

The advanced Practitioner role is to provide specialist interventions to clients in the ICMP service in Greenwich.

Post holder will be an experienced professional who aims to provide a safe, effective, caring and responsive service to patients.

Post holder will promote core assessments of caseload; to include history taking, clinical assessment, impression/diagnosis, treatment plan and evaluation of care.

Post holder will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within the ICMP Service.

The post holder will use a holistic approach to consider interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.

The post holder will participate in, and influence efforts, across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure appropriate support through proactive provision of care in the community.

The role will include the provision of strong leadership to the team.

Main duties of the job

  • Offering expert clinical practice/care to clients in the Intensive Case Management of Psychosis (ICMP).
  • Supporting leadership and management, in order to improve and transform the service.
  • Supporting research, audits and evaluation - applying existing research and audits and contributing to new research within chosen speciality.
  • Supporting supervision of staff within the team.
  • Education and professional development of self and others.
  • Delivery of NICE concordant interventions
  • Planning, Coordinating and evaluating carer focussed education and support programmes
  • Contributing to provision of a culturally appropriate service for service users, carers and families
  • Provision of consultation to team members.
  • Keeping updated in research/ changes to NICE guidelines and evidence-based practice.
  • Use of routine outcome measures.
  • Monitoring and reporting on the provision of NICE recommended interventions across the ICMP teams.
  • Lead in ensuring practice in the ICMP team reflects Trust Carers strategy.

Clinical:

  • To work autonomously in processing referrals, undertaking assessments, selecting appropriate clients with complex needs who are suitable for psychological treatment and interventions.
  • To deliver highly specialised Intervention treatment for clients presenting to the ICMP team.
  • To refer clients on, as appropriate, to other statutory and non statutory services.
  • To work with clients in a variety of settings, including the clients home and a range of community settings.
  • To use specialist measurement tools and rating scales in undertaking highly specialist psychological assessments, formulation of specialist care plans and monitoring progress and measuring treatment outcomes.
  • To assess clinical risk and formulate management plans, including crisis and contingency plans.
  • To be directly accountable for own professional action, quality of clinical intervention provided and the maintenance of high standards of care within professional codes of practice, professional ethics and legal standards.
  • To develop therapeutic relationships incorporating limit and boundary setting and therapeutic confrontation when inappropriate behaviour is exhibited.
  • To provide culturally appropriate interventions with carers and families of referred clients.
  • To attend MDT meetings and be an active member of the team giving psychological input into case management and providing advice on Interventions to other professions.

  • To contribute to the monitoring and evaluation of psychological interventions across ICMP
  • To plan, coordinate and evaluate carer focussed education and support programmes.
  • To contribute to the consultation and engagement of service users, carers and families in planning and delivering services which meet the needs of local communities.
  • To attend relevant meetings and working groups to ensure practice in the ICMP team reflects the Trust carers strategy.

IT responsibilities.

  1. To regularly enter and update information regarding clinical activity into the RiO clinical records system.
  1. To competently utilise Microsoft packages including Word, Excel and Power point in order to produce professional clinical and training materials for use as part of normal clinical responsibilities.

  • To ensure that up to date written and electronic records and activity data are maintained in accordance with Professional and Trust standards.

Manage own Workload:
