Community Support Worker/Discharge Facilitator, Band 3 Base: Greenwich ICMP, 68 The Heights, Charlton SE7 8JH
An exciting opportunity has arisen for a post within the ICMP Team in Greenwich, London. This is a 37.5 hour per week post as a discharge facilitator within mental health services.
We are seeking an autonomous, enthusiastic, practitioner to join our innovative team. The post holder will be working within a multidisciplinary team to review the discharge of service users within the team, whilst participating with the ongoing development of service delivery. You will be expected to work collaboratively within a multi-disciplinary team to deliver support to working age adults.

Main duties of the job
This post is based in the ICMP community mental health services within the London Borough of Greenwich.
The Greenwich locality consists of a number of functions:
Primary Care Plus (PCP) for new referrals and shared care withGPs for people with episodic mental illness; affective disorder and personality disorder treatment pathway (ADAPT) and a psychosis treatment pathway (ICMP).
The post holder will work closely with The MDT, GP/ PCP and otherservices to support discharge of patients from the ICMP team. The Postholder will work with the MDT to support discharge back to primary care.Working as an active member of the multi-professional team, in providing care and treatment to service users primarily on Care Programme Approach (CPA) is an integral part of the role.
