Job summary
We are seeking a highly experienced, motivated and forward thinking Community Mental Health Nurse for our Bexley Community Mental Health Rehabilitation and Enablement Service (BCMHRES ) team.
You will have previous experience of working in similar environments and be able to demonstrate you have the requisite skills to provide excellent clinical care to our service users, carers and families.
The BCMHRES Service within Oxleas NHS Foundation Trust offers a specialist multi-disciplinary service for individuals suffering with and recovering from severe and enduring mental health problems. We utilise a planned and integrated approach to care that is delivered in conjunction with community, in-patient, crisis and other specialist services.
We provide Intensive Support for working age adults with severe & enduring mental illness associated with significant disability, predominantly psychoses. Our service-users present with disorders requiring skilled, frequent and intensive intervention including psychological interventions, medication management, occupational therapy, supervision of complex problems of management and engagement.
We work collaboratively with service users, carers and family networks to actively pursue their goals and aspirations while minimising the impact of their long term mental health difficulties.
Main duties of the job
- Promoting of independence to enable clients to move to independent or less supported accommodation
- Working with third sector providers in monitoring, reviewing and delivering care as planned to facilitate movement of clients in supported living establishments
- Enabling service users to manage their mental health in conjunction with Gps external to the secondary mental care services
- Working collaboratively with service users , carers and their family networks to attain their set goals and aspirations and minimising the impact of long term mental health difficulties.

Foster recovery through empowerment of service users and carers by involving the service user and carer in the planning of their care establishing and maintaining consistent supportive relationships, which encourage increased self-determination and responsibility for service users and their care.
- Offer a range of evidence-based bio-psychological interventions and risk management approaches enabling the service user and carers to reduce and manage symptoms and high-risk behaviour.
- Be proactive, innovative and creative in delivering care, enabling the facilitation of service users recovery.
- Engage service users and carers in relapse prevention and crisis plans, providing intervention that maximizes the service users ability to resolve crisis and prevent hospitalization.
- Facilitate early discharge by providing regular, formal, joint in-patient reviews.
- Deliver a flexible and responsive service through practice that is dictated by service users/carers needs.
- Work collaboratively and effectively with members of the multi-disciplinary team and in partnership with other agencies.
- Deliver high quality care and provide solutions to service users with severe and enduring mental health problems.
- Enhance social functioning, maintaining service users within their own environment in the community.
- Act as an educator for service users and carers. Advise on issues of mental health so as to generate a positive approach to health and strengthen the service users capacity to manage illness, symptoms and crisis.

- Carry out supervisory duties as required under Section 25 of the Mental Health Act 1983.
