Job summary

The Senior RTT Pathway Tracker role includes management of the patient journey through the care pathway in accordance with NHS England Referral to Treatment (RTT) guidelines and requirements.

The successful candidate will have good organisational skills and a good knowledge of the RTT 18 guidelines. They will work closely with the Operational Management Team and liaise with the administrative and clinical staff to progress patient pathways and ensure accuracy of waiting lists.

This is an office based position.

Main duties of the job

1. Job Summary

1.1 To provide accurate data on possible and actual breaches to the clinicians, managers, specialist nurses and secretaries and assist in reviews of pathways to avoid breaches. The post holder will validate, resolve and correct Referral to Treatment (RTT) data quality issues

1.2 The post holder will be readily available to offer guidance and to support the training of clinical and administrative staff involved in patient pathways in the correct procedures and processes relating to RTT. To provide consistent guidance to all staff relating to the RTT procedures and processes. To deliver presentations on RTT to all staff as required.

1.3 The post holder will have strong working relationships with specified admin teams and will therefore build self-knowledge of speciality and sub-speciality clinical pathways within those specialties.

To monitor and provide guidance and training, to clinical and administrative staff on the use of RTT status codes, ensuring their accurate use by all staff members.

To present information regarding RTT processes to all levels of staff, clinical and administrative, adapting complex RTT data as appropriate for the target audience.

To escalate problems with capacity with relevant staff and work with colleagues to implement solutions to capacity issues.

To use multiple clinical information systems and databases to understand and coordinate patient pathways.

To proactively identify and support services in the resolution of pathway delays.

Using specialist knowledge, to update RTT pathways where information is identified that has not been included on the pathway which may affect clock start and stop dates.

Have autonomy to investigate and validate patient pathways in liaison with services, using own initiative to ensure breaches are avoided. This would include productively challenging the progress of patient pathways with relevant staff members.

To be responsible for ensuring that patient pathways are correctly linked and reflect the patient journey.

To highlight bottlenecks with the patient pathway, communicating regularly with service management and where possible suggesting and implementing solutions.

Any other duties appropriate to the post-holders grade. Staff may be required to undertake any other duties at the request of the line manager, including project work.

To ensure that incoming referrals are accompanied by accurate and timely pathway information and that this is being recorded correctly.

To deputise for the RTT and Referrals Team Leader, in relation to data reports, if required.
