Burns Outreach Specialist Nurse Practitioner

Job summary
Act as the Trust's nursing clinical lead for the Burns Outreach service. Providing expert tailored treatment, advice, consultation, support and guidance to patients, carers and referring trusts whilst working as a highly skilled, expert, advanced and autonomous practitioner liaising within multi- professional teams to promote and provide seamless pathways of care. The post holder will work closely with burns consultants to manage patients appropriately to gain support and advice to ensure continuity of care.
Evaluate current treatments to improve clinical outcomes improving quality, health outcomes and service delivery. Work collaboratively with internal and external agencies across professional and organisational boundaries and the local health economies.
Initiate and facilitate audit and research that aims to collect evidence to support and strengthen clinical practice and to enhance knowledge and understanding of wound healing.
Develop the service for the future ensuring sustainable growth whilst maintaining standards of specialist care. Innovate and change current practice through the delivery of appropriate education and training to meet service needs. Attend and regularly contribute to the Burns Governance meetings.
Driving licence is required for this role.
Main duties of the job
Work as part of the Burns Multi-disciplinary Team (MDT), whilst reporting to the Burns Advanced Clinical Practitioner, and being responsible for ensuring that any outreach burns patients are discussed regularly at the relevant MDT meetings. Maintain the professional relationship already forged with the Plastics Outreach Team, to develop both Burns and Plastics clinical skills and knowledge and ensure peer support is available to assit in times of high caseload numbers or complex patients. Work closely with burns consultants to manage patients appropriately to gain support and advice to ensure continuity of care.
