Main duties of the job
The Frailty Directorate is undergoing as exciting expansion with the development of a Frailty 7-day Same-Day Emergency Care service and Rapid Access Care of the Elderly clinics. This role would be supporting the development of an Integrated Frailty Medicines Optimisation Pharmacy Network.
We as a team are keen to focus on continuous improvement of our services for the benefit of our patients and welcome new ideas and opportunities. We aim to continuously upskill and develop our workforce and currently pharmacy assistants on ward areas supporting with medicines management including dispensing on wards, and medication supply checks for discharges, improving safety and reducing waste.

As part of multi-disciplinary team within the frailty team providing a 7 day patient facing clinical service to support Medicines Optimisation within the trust in an exciting new Frailty Pathway, including partaking in research opportunities.
Providing safe and effective use of medicines to enable the best possible outcomes, reduce inappropriate polypharmacy & improve adherence and patients understanding of medicines
Together there will be structures to expand the team, work on improving patient flow and optimising patient care through medicines management across sectors. This will include involvement in post-take ward rounds, clinics and joining up care with our interface, Primary Care Network (PCN) and Medway & Swale Medicines Optimisation teams. The Clinical Pharmacy Frailty team also provide and deliver training, undertake clinical audit and service evaluations, contributing to service planning and development.

Qualifications
Essential
- Vocational Master's degree in Pharmacy and successful completion of pre-registration training and experience
- Registered with the General Pharmaceutical Council as a Pharmacist in the UK (or eligible to be)
- Postgraduate Diploma or equivalent (or be working towards one)
Knowledge

- Understanding of frailty syndromes and principles of medicines optimisation
- Good core therapeutic knowledge
