We are recruiting for the SMART (Surgical Medical Acute Recovery Team) Service, a Clinical Sister / Charge Nurse, who shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take the service and the Trust forward, by ensuring that we continue to be the first choice for patients.

Main duties of the job

As a Clinical Sister/ Charge Nurse you will provide clinical and professional leadership, supervision and support to patient, nursing staff and students within the SMART team.

You will triage, assess, plan and implement the care of patients who are transferred home under the care of the SMART service, into a H@H virtual ward in the community. We use Remote patient monitoring, hence a good clinical knowledge in analysing vital signs and escalating clinical concerns for urgent treatment via our virtual pathway is essential

Responsibility for reviewing care delivery and maintaining records in accordance with local and with NMC guidelines.

The work will include driving to visit patients in the community therefore it is essentialthat you have a full UK driving license and a car

Job responsibilities

Members of SMART work autonomously, therefore we are looking for someone with excellent evidence based experience in the care of acute patients. Specific training will be provided to facilitate the pathways covered by SMART in the community. Confidence, good communication skills and the ability to make sound decisions is vital.

The Clinical Sister / Charge Nurse must also be able to liaise with all members of the multidisciplinary team to coordinate safe effective and timely care practices within the SMART service.

The successful post holders main responsibility will be the day to day care of patients on SMART h@h virtual ward, and also ensuring that all patients referred are triaged and prepared safely to continue their inpatient treatment in an outpatient setting.

The SMART Service is nurse led, therefore the ability to work autonomously as well as effectively within a team is essential to initiate prescribed care in conjunction with consultant / other clinicians without direct supervision, reporting directly to the Consultant/team manager/clinical lead and undertake care delivery involving an extensive range of activities acquired through experience or additional short courses e.g acute and chronic disease awareness/management, assisting the MDT, acute/chronic pain management, appropriate care of wound assessments and management, blood monitoring/understanding the results for health promotion. Act as mentor to students, adopt link roles, sound knowledge in intravenous drug infusions, fluid therapy, administration of nebulised drugs, administration of oxygen therapy, computer skills, liaising and with other agencies and professionals, observation recording and interpretation, venepuncture, cannulation, management of central and mid lines, assessing patient and in depth history taking

Assess, plan and evaluate care provided for patients including those with complex needs

As a SMART nurse teaching, auditing, infection control practices and undergoing research are activities undertaken within the role.

Qualifications

Essential

  • RN1/RN2/RN12 or RN Child Registered Nurse on the NMC register. To hold the appropriate level for the area of speciality
  • BSc / 1st degree (health related) or equivalent qualification / training / experience

Knowledge

Essential

  • Foundation level of understanding regarding the acute care needs of patients
