Main duties of the job
The fundamental role of the Pathway Coordinator is to provide a full and confidential administrative service to the clinical teams. Patients must be managed on their pathway from referral to treatment, ensuring all relevant information is available and that the recording of information relating to clinical episodes for individual patients occurs.
The post holder will need to understand the targets relating to Choose and Book and the requirements of the 18 Week Access Policy.

Ensure that appointments are made within the correct timescales and any follow-up actions from clinical appointments are carried out, i.e. tests or appointments ordered and bookings made within the correct timescale. Ensuring any capacity issues are escalated to the service team
Develop systems to ensure rapid communication and effective co-ordination between all other Trusts/Centres where patients may be referred for specialised diagnostic tests or treatment
Act as a tracker for an identified cohort of patients on the 18-week pathway so that they are not lost in the system. Be responsible for ensuring patient data is correct on PAS, i.e., ensure clinics are cashed up and the correct outcome entered the procedure codes and RTT codes.
Add all new patients to the Patient Tracking list.
Ensure that patients are accurately monitored through each stage of their diagnostic and treatment pathway
Be responsible for ensuring patient data is correct on PAS, i.e., ensure clinics are cashed up and the correct outcome entered the procedure codes and RTT codes
Check patients demographic information is correct and updated to ensure correspondence is sent to the correct address
Liaise with all relevant departments to ensure that all tests results, case notes and other relevant information are available for clinical appointments
Take responsibility for identifying potential breaches of the access and treatment waiting times targets
Communicate such data to appropriate members of the clinical team, Service Team or the General Manager. Whilst it is a role of the co-ordinator to ensure that the appropriate clinical teams and Service Manager are aware, it is the managers responsibility to take the necessary actions to avoid all breaches
Provide administrative support to their speciality team.
Contact is required with patients parents / carers on their pathway, excellent telephone skills required and empathy is essential
To be actively involved in process mapping and capacity & demand studies when required.
To work with the team to develop processes and procedures to maintain and improve performance against the access and treatment pathway targets
To ensure patients are treated without process delay
To undertake any work that their team may require support for that is appropriate to their skills and knowledge
Assist in the introduction and use of new technology and office procedures as they arise
Train staff in new technology and procedures if required
Maintain efficient and safe office working practices
To be willing to work flexibly with other members of the Admin & Clerical team to provide cover and support
To provide cover for team colleagues during periods of absence, co-ordinating leave booking to facilitate this
To learn and understand the roles of all other team members with a view to the team providing a seamless and comprehensive service during periods of absence
