Job summary

We are seeking an experienced and motivated Band 6 Community Psychiatric Nurse to join our dynamic and supportive Locality Mental Health Team. The post-holder will provide high-quality, evidence-based care to adults with complex and severe mental health needs, working both autonomously and as part of a multidisciplinary team.

As a key member of the multidisciplinary team, you will provide autonomous clinical assessment, risk management, and evidence-based interventions, working collaboratively with service users, carers, and partner agencies. You will hold a defined caseload and act as a lead professional ensuring personalised, holistic, and trauma-informed care planning.

The role requires strong clinical judgement, excellent communication skills, and the ability to work flexibly in a fast-paced environment. You will contribute to the team's duty function, undertake initial assessments, and support safe and effective care planning. You will also play an active role in service development, quality improvement, and promoting a culture of learning and reflective practice.

Main duties of the job

Provide comprehensive mental health assessments, formulation, and evidence-based interventions for adults with complex and severe mental health needs.

Act as a lead professional for an allocated caseload, ensuring high-quality, person-centred care.

Undertake ongoing risk assessment and risk management, including the development and review of crisis and contingency plans.

Deliver a range of therapeutic interventions, both individually and in groups, in accordance with clinical guidelines and service pathways.

Administer medications in the community, including intramuscular injections, as per treatment plans and prescriptions.

Participate in the team's duty rota, responding to urgent calls from patients, carers and partner agencies.

Work collaboratively with the wider multidisciplinary team, primary care, social care, voluntary sector partners, and carers to ensure coordinated and holistic care.

Maintain accurate, timely, and professional clinical documentation in line with Trust policies and regulatory standards.

Contribute to service development, quality improvement initiatives, and the promotion of safe, recovery-focused, and trauma-informed practice.

Provide supervision, mentorship, and support to junior staff, students, and colleagues as required.

Deputise for the Team Manager as necessary.

Uphold professional standards, safeguarding responsibilities, and adherence to Trust policies, clinical governance, and NMC requirements.

Plan, implement, review and improve interventions to meet peoples identified needs and manage their inherent risk.

Enable individuals to develop independent living skills and live in the community.

To work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, both within the Community Mental Health Team and with external agencies.
