Senior Healthcare Support Worker

This is a unique opportunity to be part of a new initiative that is one of its kind in the country. Reablement is all about working closely with our service users to live as independent a life as they would like, in a time frame that is appropriate to their individual needs. Working collaboratively with our service users, taking a 'hands on' and 'Doing it with them, not for them' approach.

The Wolds sits within rehabilitation; we are a 16-bed, mixed-sex ward with individual rooms and en-suites. We are very much a multi-skilled and diverse team who delivers excellent care to our service users and is inclusive to family or carers during the inpatient stay.

You will be working alongside the MDT to support our service users to achieve an independent life, supporting staff nurses with reviewing the service users' individual and personalised care plans, working closely with our social inclusion workers to better support our patients, as well as facilitating the groups specific to our clients' needs.

Main duties of the job

To work alongside the nursing team to support our patients through their reablement pathway, working alongside the MDT to support our patients to achieve an independent life, you will be supporting the nurses with reviewing patients' individualised care plans, working closely with the Social Inclusion Workers to better support our patients' needs.

Duties include conducting observations, physical health checks and documentation; supporting patients with their ADL's including, facilitating escorts off ward; IROC using the recovery model; psycho education and life skills. Preparing our patients with the essential skills so they can use these to live their lives comfortably in the community. You will have the opportunity to have monthly managerial and clinical supervision and annual appraisals to identify the areas you wish to progress in to further your career.

Desirable

  • Voluntary or working capacity with mental health service or equivalent

Qualifications

  • NVQ Level 2 in Care or equivalent
