Job summary
We have an excellent opportunity for a highly motivated individual to join our established Older Adult Community Mental Health Team. Excellent communication and interpersonal skills are necessary for this post, with adaptability being essential as no two days are the same.
As an assistant practitioner, you will be conducting memory assessments at our office base, within care homes and within people's homes within the locality, and subsequently taking these assessments through the process of diagnosis and treatment pathways where suitable. You will be monitoring the titrations of the anti-dementia medicines and when to escalate any concerns. Annual monitoring of these medications will also be completed, via the telephone and face to face. Reviews on behavioural and psychological symptoms of Dementia will also be a big part of your day-to-day role.
Liaising with various stakeholders will be a regular part of your job; this can include GP surgeries, carers and families, other teams within LPFT, and Adult Social Care. Regular joint working with other teams within LPFT, such as the Home Treatment Teams and the inpatient wards, requires some flexibility and shared responsibility.

Main duties of the job
- To provide a high standard of care and comprehensive assessments to those experiencing mental health difficulties, whilst working within the Trust's values
- To monitor titrations of anti-dementia medications and complete annual reviews of anti-dementia medications used within the CMHT, and subsequently complete relevant documentation
- Complete memory assessments at base, within people's homes, and within care homes in the Grantham/Sleaford locality, and complete relevant documentation.
- Supporting individuals to develop independent living skills and live well at home
- Supporting individuals in accessing the right services for their needs
- Review people struggling with their mental health, both functionally and with a diagnosis of dementia.
- To manage a caseload and service delivery to contribute towards the Trust's vision of keeping people well at home within the community
- To work collaboratively and promote effective working relationships with members of the multidisciplinary team, ensuring effective and appropriate clinical decision-making, both within the CMHT and with external agencies.
- To assess carers' and families' needs and develop, implement and review programmes of support for carers and families.
