Job summary

Boston older adult CMHT offer an exciting opportunity to a candidate who has a passion for supporting our patient group with a variety of mental health needs. We offer mental health support for over 65's and an all-age memory assessment and monitoring service.

We are looking for someone who has previously worked as a health care assistant in a health or social care environment and wishes to further their career in older adult mental health services.

This role will include support to patients on both the mental health and memory assessment pathways, reviewing and monitoring patients on medication and sharing those findings to qualified practitioners, supporting patients with planning end of life care via the Respect agenda, providing group work to support our patients to maintain their wellbeing, and supporting patients by monitoring their physical health and wellbeing.

Training will be provided to support this role, with continuous training across the healthcare system being available throughout the year.

Main duties of the job

Monitor patients newly diagnosed with dementia as they begin medication. Communicate to the MDT/qualified practitioner the patients tolerance of medications to enable decisions in regard to titration.

Provide ongoing monitoring of said medications, following NICE guidance. Communicating any changes in presentation and risk to the MDT/qualified practitioner to ensure that appropriate decisions can be made in regard to their treatment plan.

Engage in use of digital technology to contact patients and carers to undertake reviews where appropriate

Work alongside Community Psychiatric Nurse/Social Worker/Occupational Therapist to plan, deliver and evaluate the approved individual and group psychosocial interventions.

Act as a key worker and case manager to service users accepted into the service and identified to be of low to moderate complexity under the supervision of a Registered Professional.

Provide support and advice and liaise/engage with partnership agencies, multi-disciplinary teams and the wider community where appropriate.

Use techniques derived from one or more therapeutic models to engage and optimise service user treatment outcomes and to support their long-term care plan.

When indicated refer or signpost on to a range of other specialist and third sector services

Work closely with, support and provide information to service user families/carers, and wider community services that support the service-user care-pathway.
