Job summary
An exciting opportunity has arisen for an experienced clinician to work within the Spalding Community Mental Health Team. The team provides assessment and trauma informed recovery focussed psychosocial interventions to working age adults experienceing severe and enduring mental illness the effetcs of which are severe and complex in nature. The team is actively engaged in the developing vision of community transformation and works closely with a range of sepcialist and other agencies across the wider health and social care system consisting of both statutory and non-statutory organisations. The CMHT seeks to provide care in a flexible and innovative way to meet the needs of the individual, their social networks and the community we serve, and the successful candidates values will be equally as important as their experience
Main duties of the job
To act as the lead healthcare professional ensuring up to date agreed care plans, risk assessments and reviews are in place.
To act at all times in a trauma informed manner, recognising the psychological and social needs of service users and those around them.
To deliver high quality, evidence based biopsychosocial interventions focussed on the service users goals, aspirations and strengths.
To Work with individuals and their wider social networks to assess their mental health, recognise mental health problems and identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour.
To work collaboratively with other members of the team, whilst maintaining overall responsibility for patients care, where appropriate.
To protect people from abuse, neglect and harm.
To assist individuals to focus on their personal strengths, recovery goals and and consequently to live a full and meaningful life.
To work collaboratively and promote effective working relationships with members of the multi-disciplinary team and wider health and social care system, ensuring effective and appropriate clinical decision-making.

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.
To work flexibly across a 7-day-a-week service.
To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate.
To ensure the clinical caseload and clinical practice of nursing members of the Community Mental Health Team is of the highest standard of clinical care.
To manage case-load and service delivery in accordance with and in order to contributetowards the attainment of Service/Trust productivity requirements.
To act as care co-ordinator under the Care Programme Approach, ensuring up to date agreed care plans, risk assessments and reviews are in place.
Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour.
Plan, implement, review and improve interventions to meet peoples identified needs and manage their inherent risk.
To assess carers and families needs and develop, implement and review programmes of support for cares and families.

Enable individuals to develop independent living skills and live in the community.
