Job summary

We are looking for a full-time Physical Health Practitioner to join our Healthcare team and provide a full range of healthcare to vulnerable children and young people resident in the home. The children and young people are between the ages of 10 and 18 and have often experienced adverse childhood events and trauma. Their everyday care is met by county council care staff and our healthcare team enhances this care by providing multi-disciplinary support for healthcare needs, including, physical health, mental health, and psychological/emotional well-being. The Physical Health practitioner will be responsible for monitoring the physical health needs of the children and young people, working in partnership with the health team, care staff and the local GP practice. They will lead on the health screening of young people on admission, ensuring all physical health needs inform each child's care plan.

The post holder will deliver training/workshops on health issues to children and young people in partnership with the unit's school and to staff on health needs/treatment as appropriate. They will work in partnership with the GP practice to coordinate the vaccination program and any prescription needs. They will also take a lead in supporting any drug and alcohol related difficulties.

The core hours are 9.00-17.00pm Monday-Friday with flexibility to support after school access to healthcare up to 21.00pm with some on-call up to midday at weekends for health screening for admissions.

Main duties of the job

To provide a high standard of physical healthcare to children and young people, within an MDT environment, including health screening, health promotion and education, sexual health, coordinate programme of immunisation and vaccination and support with identified substance misuse as appropriate.

The post holder will be responsible for conducting regular health checks for children and young people resident in the unit, as well as undertaking physical observations and procedures as required. In addition the postholder will provide sessions for long-term conditions such as asthma and diabetes, coordinate secondary health needs and facilitate the GP clinics.

The post holder will provide professional advice and support to members of the multi-disciplinary team and work flexibly between 9am and 9pm as required, including some on-call duties at weekends and bank holidays. The post holder will deliver group work and individual care packages to children and young people resident in the LSU with physical health and/or substance misuse problems.

Work with the visiting GP/ GP Nurse/ Dental, optician, Sexual Health Services and community Immunisation Teams to provide an effective primary care service.

Ensure that families/carers are included in the planning, implementation and evaluation of care.
