Job summary

The Stamford Community Mental Health Team are looking to recruit a full-time band 5 registered Mental Health Nurse.

This role gives the opportunity to be involved in the journey of transforming adult community mental health services within LPFT. It offers the practitioner opportunities for creative approaches to working, ensuring the patient remains central in the care and intervention planning. Support and interventions are delivered that are recovery focused and promote independence and resilience and deliver individual needs led, high quality outcomes for patients and carers. You will work co-productively within Lincolnshire healthcare including Primary and Secondary care services, social and voluntary care system and patients, and carers. This enables individuals to achieve their optimum physical, psychological, and social wellbeing and improve the lives of people who access healthcare services in the locality of Stamford and Bourne

Main duties of the job

The main duties of the role will include:

To act as lead professional and coordinate care as appropriate, support service users with complex mental health needs demonstrating practice that is evidence-based and innovative.

To provide comprehensive mental health assessment, formulation and collaborative care planning with service user and carer/family, under the supervision of the line manager.

To formulate risk management plan with service user and carer/family, under supervision of the line manager.

In conjunction with the multi-disciplinary team, will provide therapeutic interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.

To attend regular MDT meetings with the team manager to address caseload management issues such as acceptance of assessments, the provision of ongoing care, discharge planning, face-to-face contacts and service user mix.

Supporting clients with their physical health awareness and some health education.

Administer prescribed medication and assess the service user for desired and non-desired effects

Provide education, advice and guidance to service users and their carers and families.

Allocate diary time to cover to the duty rota.
