We are pleased to be able to offer this exciting opportunity join the S75 Social Care Service in Lincolnshire. Lincolnshire Partnership Foundation Trust provide this service on behalf Lincolnshire County Council through a partnership agreement under S75 National Health Service Act 2006.
The Social Work Service is embedded in the Community Mental Health Teams, but offers a distinct service with professional social work line management structure. Assessment, short term and long term social work interventions as well as support planning and reviews are all carried out in accordance with the Care Act 2014. Partnership working with health colleagues gives the opportunity for contributing to the provision of a specialist and integrated service for service users.
The service currently has a Social Worker post within the Social Care team, within the West team. The West team cover areas of Lincoln and Gainsborough.
The Post is advertised at Band 6, with opportunity to take up the post at Band 5 for newly qualified staff. For any successful candidate appointed at a Band 5, where applicable, ASYE will be offered and progression to Band 6 upon successful completion of this.
As a service we hold quarterly social work development days to assist with continued professional development. Peer supervision groups meet on a regular basis and one to one regular social work led management supervision will be provided

As a social worker you will develop your core skills towards becoming a more competent practitioner within a specialist service through the guidance of the National and Local competency frameworks

Ensure evidence-based practice and research awareness is used as the foundation for the assessment, planning implementation and evaluation of nursing care.
To engage with service users and their families/carers, developing therapeutic relationships, and providing evidence-based, psychosocial interventions in a variety of settings'
Act as care co-ordinator for an agreed number of service users, adopting the team approach to provide interventions to other service users, families/carers as appropriate.
Assess complex health and well being needs, develop, monitor and review care plans to meet specific needs.
Undertake initial and ongoing risk assessments, utilising standardized tools, in order to develop accurate and appropriate risk management plans and intervention, highlighting service user strengths, and positive risk options.
Undertake or facilitate Carer's appraisal of need as appropriate.
Develop and co-ordinate integrated team working with team, and other agencies involved in the care of the individual, their families/carers.
Promote service user and carer involvement; ensuring choices are embedded into the philosophy of care.
Empower people to address specific needs in relation to health and well being, and enable individuals to become expert in managing their condition.
Ensure social work perspectives and values are supported and represented within the multidisciplinary approach.
Keep up to date with current practices and issues in Mental Health and Social Work.
The post holder will be expected to undertake Approved Mental Health Professional Training, if not already qualified as such.
