Job summary

We have an exciting opportunity to join the Grantham Placed Based team which is part off Lincolnshire Partnership NHS Foundation Trust's Adult Community services. The IPBT work closely with the wider Neighbourhood as part of the local transformation across the local primary care network. The Nursing Associate role is relatively new to the NHS and we are looking forward to you joining us in forging the way for Nursing Associates in our workforce. The role will be part of an integrated health/social care team providing care for people experiencing mental health problems in the local community. Offering nursing interventions in a wide variety of clinical, therapeutic and social forms with the aim of promoting health, wellbeing, self-autonomy and independence.

Main duties of the job

The IPBT are part of the developing transformation service. The post holders will be required to work effectively as part of the multidisciplinary team in the delivery of high-quality mental health care planning and risk management. This work will involve patients and their careers in the delivery of evidence-based interventions/treatments, recovery focused work. The post holder will have a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person-centred care and support within the adult community setting. The nursing associate will work independently under the leadership of the registered nurse, working within the sphere of nursing and care and within all aspects of the nursing, providing holistic and person-centred care and support within the adult community setting. Monitor the condition and health needs of people within their care on a continual basis in partnership with service users, families and carers
