Job description

Job responsibilities

• You will attend clinic, counsel patients and order and review blood tests before patients are initiated on immunosuppressant medicines.
• You will co-ordinate ongoing blood test monitoring whilst patients are using immunosuppressants.
• You will work with medical leads in multiple services to formulate patient care plans for those with out of range blood test results.
• You will attend MDTs with internal and external medics.
• You will contribute to medication and financial governance workstreams.
• There will be opportunity to undertake audit and present your work.

Person Specification

Education & qualification

Essential

• Postgraduate Certificate in Pharmacy Practice or equivalent experience

Desirable

• Postgraduate Clinical Diploma/MSc in Pharmacy Practice or equivalent experience.
• Non-medical prescribing qualification

Experience

Desirable

• Specialist clinical pharmacist experience within the hospital setting.
• Clinical guideline/protocol development
• Clinical audit
• Management within hospital of patients supplied medicines via homecare services
• Experience in providing pharmacist support to ophthalmology services

Knowledge

Essential

• Medicines optimisation
• Medicines regulation

Desirable

• Ophthalmology
• Management of chronic inflammatory diseases

Skills / abilities

Essential

• Be able to provide, interpret and present highly complex information on medicines use to clinical and management leads
• Be able to provide highly specialised information on medicines use to other healthcare professionals.
• Be able to communicate effectively in both written and verbal formats with staff and patients.

Desirable

• Be able to act as a mentor to junior staff
• Be able to influence senior medical staff, the multidisciplinary team and management.
