Job summary

The post holder in collaboration with the Matron, Lead Nurse, Advanced Nurse Practitioners and the Multi-disciplinary team is responsible for the delivery of safe, effective care in Rapid Access Clinic and/or Urgent Care Clinic leading to an excellent experience for patients and carers by;

  • Being responsible for ensuring high quality, evidence based patient centred care is delivered and providing assurance of this whilst delivering the Trusts core objectives and strategies
  • Being responsible for setting the standards with the nursing team and ensuring that the environment allows the workforce to feel supported to deliver safe and effective care
  • Developing and delivering continuous improvement initiatives to ensure that the service meets all Quality Indicator targets, Key performance indicators and quality and safety standards and initiatives relevant to the service
  • Reviewing and implementing the management of patient pathways to improve flow and efficiency in the clinics/departments, ensuring the patients experience is positive while under our care

Main duties of the job

Clinical Practice

  1. Practice autonomously with a high level of accountability at an advanced clinical level in the direct delivery of high quality care, making critical clinical decisions based on advanced clinical skills assessment.
  2. With the support from the Nurse Consultant, Advanced Nurse Practitioners (ANP's), Lead Nurse and the Medical team, be responsible for a caseload of patients managing nurse led clinics.
  3. Works in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. history-taking, holistic assessment, identifying risk factors, mental health assessments, requesting, undertaking and/or interpreting diagnostic tests and conducting health needs assessments).
  4. Collaborate with the multi-professional team to establish diagnosis and monitor disease progression in patients with ophthalmic conditions.
  5. Challenge practice so that the best and most appropriate treatment is given to the patient; maintaining and monitoring high standards of care, recognising and responding proactively to patients' changing clinical needs.

To work as an advanced clinical practitioner to maximise the value of medicines with regards to medicines optimisation in adults and paediatrics across all services.

Managing referrals of patients for a pharmacy-led review of their medicines and -maintaining appropriate databases, dosing histories and any necessary communication to primary care/community services and that all relevant information is also recorded in the Trust system such as OpenEyes or Medisoft. To be an acknowledged source of expertise in the safe and appropriate use of medicines throughout the Trust and the wider health community regarding medicines optimisation, digital, genomics and personalised medicine. Provide support and advice to other healthcare professionals and organisations regarding the optimising medicines in ophthalmology and ophthalmic services/pathways.

Lead the development and review of Trust policies, protocols and guidelines relating to medicines optimisation including digital, genomics and personalised medicine based on research evidence, national guidance and accepted best practice.Advise on new therapeutic (medicine) developments and/or service delivery and/or pathways related to optimising ophthalmic medicines and how these will impact on the Trust and wider health community.

To become a recognised regional / national expert in the pharmaceutical care of patients using ophthalmic medicines with regards to optimising the use of those medicines. This will be achieved through participation in appropriate working groups, national specialist groups, conferences, educational events and publications, contribution to national policy and practice development.

To work with the clinical and non-clinical leads for adult and paediatric patients to agree, develop and initiate new services and/or processes and/or pathways to achieve medicines optimisation such as dedicated pharmacy-led clinics as part of the patient pathway within the clinic setting, and/ora referral processes to pharmacy for medicines reviews for vulnerable patients, and to agree clinic session rosters including cover for any such clinics developed.
