Job summary

This post is based at the Ealing satellite site Daycare and Outpatients within the Northwest Directorate. Moorfields is one of the world's leading eye hospitals, providing expertise in clinical care, research and education.

PROBATIONARY PERIOD

This post will be subject to a probationary period of 6 months.

During the probationary period, your suitability for continued employment will be assessed and provided a satisfactory standard is achieved and maintained, your employment will be confirmed.The probationary period may be extended or your employment terminated either during or at the end of probation, in line with the trust Probationary Policy. For the duration of the probation period, the notice period will be in line with contractual obligations on either side.

Main duties of the job

The post holder is responsible for assessment of care needs of patients, planning programmes of care, implementation and evaluation of these programmes with direct supervision.

At times they will have leadership responsibility for being in-charge of the ward and supervising junior members of staff. The post holder will be a key member in relation to the development of practice on the ward. They will be working independently/autonomously and at times without direct supervision, accountable for their own professional actions.

Ensuring the safety and well-being of service users, and always working in a manner that promotes dignity and human rights through the adoption of person centred care principles.

Job responsibilities

Clinical and Patient Care

  • Responsible for carrying out clinical ophthalmic practice within designated clinical areas, ensuring that high quality, current evidence-based nursing assessment, care planning, implementation, interventions and evaluations for patients is provided from admission through to discharge.

  • Will be responsible for the application of knowledge across the range of ophthalmic work procedures and practices, underpinned by theoretical knowledge and practical experience.

  • To act as advocate for patients within the area, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.

  • To ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.

  • Recognise the significance of observations made and use them to develop and initiate nursing assessments and devise a plan of care.

  • Carry out those activities necessary to conduct a comprehensive assessment of a persons nursing requirements.

  • Carry out ophthalmic nursing procedures and treatments, checking on and maintaining the highest possible standards.

  • To take a clear and concise history from patients by assessing their health and well-being and complete documentation ensuring that all entries are accurate and legible and that all information systems are maintained.

  • Undertake prescribed physical observations as follows measuring Intra ocular pressures after competency-based training.

  • Performing Fundus fluorescein angiography and cannulation after being signed off competent to do so.

  • To work in the Eye theatre reception, ensuring the surgical pathway is running smoothly and on the day cancellations are prevented. Keeping the patient experience and safety the key focus of your practice. Working alongside the theatre team to ensuring good communication. Liaising with the inpatient team to maintain up to date information is feedback to day-care team.

  • Responsible for the correct administration of prescribed medication.
