Job summary

The Diagnostic Failsafe Officer is responsible for ensuring that all patients that have been seen in diagnostic clinics for the Glaucoma or Medical Retina Services at City Road & Hoxton are reviewed and actioned within the service specific timeframes. This will involve using available reporting and tools to ensure patients clinical reviews are appropriately allocated and patients are not missed.

The Failsafe Officer will need to work with all internal stakeholders to make them aware that the service specific SOPs are followed, and train staff as and when needed.

The successful candidate will support operational teams if needed to support efficient running of diagnostic pathways.

PROBATIONARY PERIOD

This post will be subject to a probationary period of 6 months.

During the probationary period, your suitability for continued employment will be assessed and provided a satisfactory standard is achieved and maintained, your employment will be confirmed.The probationary period may be extended or your employment terminated either during or at the end of probation, in line with the trust Probationary Policy. For the duration of the probation period, the notice period will be in line with contractual obligations on either side.

Main duties of the job

The Failsafe Officer will need to ensure that all health professionals and stakeholdersassociated with the service are aware of their failsafe responsibilities, and will be required tohighlight any areas of concern to the relevant Service Manager.

In conjunction with the Glaucoma and Medical Retinaservice, the post holder will ensure that the virtuis continuouslydeveloped and delivered in accordance with requirements outlined by the NationalScreening Committee (NSC) to the population of people with diabetes in and aroundLondon. The post holder will have in-depth knowledge of service management within diabeticretinal screening, assisting the Screening Lead in matters relating to the0 delivery and qualityassurance of the service.

  • Ensure have excellent knowledge of various virtual pathways for both Glaucoma and Medical Retina
  • Awareness of potential obstacles to virtual reviews/appointments including IT, software packages, duplicate records, uploading delays.
  • Use of failsafe and exception reports reporting tools, including Magic XPA, to ensure patients are safely processed through diagnostic pathway.
  • Provide RCAs for service managers in case of incidents and complaints.
  • Weekly failsafe reports to Service Directors and Operational Leads.
  • Prepare and present to relevant service level meetings.
  • Regular meetings with admin teams to make them aware of potential KPI breaches and support with implementing processes that prevent delays.
  • Flag reviews that are missed and monitor if patient letters have not been sent to patients within agreed timeframes.
  • Create and share training and documentation for successful management of virtual tabs on OpenEyes.
  • Monitor the virtual tabs for incorrect site referral and let relevant teams know.
