This is an exciting role within the Diabetes and Endocrine team for a Familial Hypercholesterolaemia / Lipid Optimisation Nurse Practitioner
This post combines the roles of Familial Hypercholesterolemia (FH) and Lipid Optimisation Nurse Practitioner and will involve cross site working.
The nurse practitioner will implement a family cascade testing, diagnostic and audit service for the identification and management of individuals with FH.
The Diabetes & Lipid Optimisation Nurse Practitioner will support the MDT to offer care to individuals identified with FH or other lipid disorders.
Acting as a professional resource to facilitate a multidisciplinary approach to the care and management of patients with FH and other Lipid disorders in an out-patient setting.
Main duties of the job
A key part of the role will be close liaison with primary and secondary care colleagues to streamline pathways, to reduce the waiting times for specialised treatments and diagnostic tests.
Working closely with the Lead Nurse and the Consultants the post holder will recommend appropriate treatments to optimise therapy for individuals with FH or other Lipid disorders.
You will develop and maintain the FH and Lipid Optimisation service.
